This icon was part of my post that you can find RIGHT HERE
1. Open a new 100 x 100 image
2. Open your image and resize it. Drag it into your 100 x 100 image.
I got:

3. In a new layer, use the rounded rectangle tool and draw over what you want of your picture.
Use the magic wand tool and select the outside of the rounded rectangle. On the layers section, select the layer on which your picture is on. Press delete. Then delete the layer with the rounded rectangle. (let me know if you need any help with that).
Now desaturate your picture (Ctrl + Shift + U). Duplicate your picture and set to screen. Duplicate your screen layer. Duplicate it again and set to overlay.
I got:

4. Now make a new layer and stamp a copy of the merged layers on it (Ctrl + Shift + Alt + E). Move it over so it is beside your first pic.
I got:

5. Now i added some tiny text by
colorfilter. You can add whatever you want.
I used:

6. Now open this gradient by me.
Gradient:

Set to screen.
And you are done!
Finished Product:
